New partnership provides post-discharge care coordination

CarePort Health, a provider of post-acute outcomes management, announced a new partnership with Baltimore-based Audacious Inquiry (Ai), a health information technology and policy company to deliver “seamless post-discharge care coordination across the continuum,” using the CarePort network with Ai’s Encounter Notification Service (ENS).

Providers using Ai will now have access to CarePort’s network made up of hospitals and post-acute providers across the country to simplify and coordinate post-discharge care, said the company in a public statement, noting that hospitals and accountable care organizations (ACOs) leveraging the partnership can receive the most up-to-date and relevant information on patients regardless of care settings, geographical location or other networks used. Care teams can now access actionable real-time notifications, analytics and reporting on post-acute outcomes from CarePort solutions.

The partnership provides any customer of Ai that operates in one of the 10 states they serve, the ability to share their data with CarePort to smooth care coordination with minimal delays and duplicative efforts.
