ISMP Report on Vaccine Errors

25% of errors are attributed to the wrong vaccine being administered.

On Oct. 16, the Institute for Safe Medication Practices (ISMP) published its 2022-2023 Vaccine Bi-Annual report.

The report said, “Though immunization is one of the greatest public health achievements, continued success relies on the quality with which vaccines are prescribed, dispensed, stored, and administered.”

With an increase in the number of vaccines available, safe use is essential, according to ISMP.

The report is an analysis of 1,987 event reports submitted to the ISMP National Vaccine Errors Reporting Program,

The most common event types include:

  • Wrong vaccine (25.2%)
  • Expired vaccine (19.8%)
  • Wrong dose – overdosage (7.1%)
  • Wrong dose – underdosage (5.3%)

The report noted, “To reduce vaccination errors and improve patient safety, healthcare providers and manufacturers should adopt and layer multiple risk-reduction strategies, including those mentioned above, to target identified system failures.”
